Holiday Camp - (For all players currently in 4th - 12th grade.)
Date: December 20th & 21st 2008
Place: Golf Links Sports Complex
2400 South Craycroft, Tucson, Arizona
Instruction: 10:00am - 5:00pm with a lunch break from 1:00pm to 2:00pm
Check in: 8:30 am on Saturday, December 20th

Lunch will not be included.


Cost:
$200.00 per camper

*For all cancelations before December 1st, there will be a $25 cancelation fee*

I hereby authorize the staff of Southwest Camps, Inc., to act for me according to their best judgment in any emergency requiring medical attention and I hereby waive and release the camp from any and/or all liability for any injuries or illnesses incurred while at Camp or in transportation to a medical facility, except for injury directly resulting from gross negligence or willful misconduct. I have no knowledge of any physical impairment that would be affected by the above-named camper's participation in the Camp program, as outlined in the brochure/website. I also understand the Camp retains the right to use for publicity and advertising purposes photographs of campers taken at camp.

Notes:

- An enrollment conformation will be sent via email.
- Reduced rates for families with two or more campers. $150.00 for second sister. Due to NCAA regulations, both sisters must be in the 8th grade or below to be eligible for the discounted rate.

- In case of rain, all attempts will be made to reschedule camp activities. No refunds will be given.

- Fees include 12 hours of softball instruction, camp T- shirt and medical coverage.
